Health Care Connect helps thousands of Ontarians find Family Doctors and Nurse Practitioners in their community

Need a health care provider? Try Health Care Connect.

Last year, we launched this innovative program to refer people without a family care provider to either a family doctor or nurse practitioner in their community.

Since that time more than 57,700 Ontarians have accessed this service. This initiative, along with others like Family Health Teams and Nurse Practitioner-led Clinics, mean that more than 900,000 Ontarians who did not have a family doctor or health care provider in 2003 now have one.

To find a health care provider, call 1-800-445-1822 (Monday to Friday, from 9 am to 5 pm; service is provided in over 120 languages) or visit www.ontario.ca/healthcareconnectto register for the program.
